Birth Certificate Upload Instructions

To verify age and make sure your child is placed in the appropriate division, Little League requires that the league has a copy of each child's birth certificate. If you have not already uploaded your child's birth certificate to your account, please do so ASAP. To upload a copy of your child's birth certificate, please follow the instructions below

Log in to your account:

Click on the Edit icon next to the player's name:

Click on the plus sign next to the "Medical Release Form" 

Click the blue button "select a file from your computer"  and locate the form on your local drive (note file types allowed):
